The Employee Health Clinic is located at 1741 W. 33rd Street, Suite 110, Edmond, OK 73013.

3. Who can use the clinic?

Employees, Retirees, and Dependents (ages 2 years and older) that are currently covered under our health plan.

4. What is the cost to use the clinic?

There is no cost to Employees, Retirees or Dependents (ages 2 years and older) covered under our plan to utilize the clinic.

6. Do I have to take the annual Personal Health Assessment (PHA) to use the clinic?

Yes. To be eligible to use the Employee Health Clinic, adults need an annual Personal Health Assessment (PHA). To schedule a PHA appointment, log in at: careatc.com/patients or call 800-993-8244

7. Can I transfer my medical records from my previous doctor to the Employee Health Clinic?

Yes, you will need to fill out a medical record release form at your previous doctor or you can fill out CareATC's form and they will send it to your previous physician for you.
